This agreement is between Beth Moore Photography (Photographer), and the individual who signs and returns this contract (Model), an independent contractor whose full name, address, and phone number appear at the bottom of this form.

1. EVENT: Model agrees to appear on Friday, April 14th, 2023 from 9:00am - 11:00am and provide modeling services for the styled shoot event hosted by Photographer.

2. COMPENSATION: Model agrees on compensation of $75.00 for the entire event, which is to be paid on the day of the event, before the Model leaves the venue.

3: IMAGES: This event will have multiple photographers taking an unlimited amount of images. There is no guaranteed amount of images that shall be provided to the Model. The host (Beth Moore Photography) agrees that any images taken herself shall be provided to the Client through an online web gallery that will remain active for a total of six months of the upload date. Turn around time for any images provided by Beth Moore Photography is on average 14 days, but guaranteed no more than 30 days. Photographs in the web gallery will be a ‘high resolution’ size good for both web posting as well as printing. 

4. POST-PRODUCTION: The Photographer/s shall retain full creative control over the editing style of the finished images. This includes but is not limited to color correction, skin softening, teeth whitening, smoothing the appearance of wrinkled clothes, Photoshop usage, etc.

5. PROPER USAGE: Any image uploaded to social media platforms of any kind by the Model must not be altered in any way, including the use of filters. Acceptable changes to the image include cropping and adding text/”stickers” to the image. Any images from this session that are uploaded by the Model must use the proper crediting of the Photographer/s and entire vendor team. A vendor list with social media handles shall be provided to the Model on the day of the event. The Photographer/s retains copyright in the photographs, and hereby grants the Model unlimited but non-exclusive rights to use or reproduce the photographs that they receive.

MODEL RELEASE FORM

In consideration of my engagement as a model, and for other good and valuable consideration herein acknowledged as received, I hereby grant the following rights and permissions to Beth Moore Photography, hereafter known as “Photographer.”  They have the irrevocable, perpetual and unrestricted right and permission to take, use, re-use, publish, and republish photographic portraits or pictures  in which I may be included, in whole or in part, or composite or distorted in character or form, without restriction as to changes or alterations, in conjunction with my own or a fictitious name, or reproductions thereof in color or otherwise, made through any medium at his/her studios or elsewhere, and in any and all media now or hereafter known, specifically including but not limited to print media and distribution over the internet for illustration, promotion, art, editorial, advertising, trade, or any other purpose whatsoever.

I specifically consent to the digital compositing or distortion of the portraits or pictures, including without restriction any changes or alterations as to color, size, shape, perspective, context, foreground or background. I also consent to the use of any published matter in conjunction with such photographs. I hereby waive any right that I may have to inspect or approve the finished product or products and the advertising copy or other matter that may be used in connection with them or the use to which they may be applied. I hereby release, discharge, and agree to hold harmless Photographer, his/ her heirs, legal representatives, and assigns, and all persons acting under his her permission or authority or those for whom he/she is acting, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of such photographs or in any subsequent processing of them, as well as any publication of them, including without limitation any claims for libel or violation of any right of publicity or privacy.

I hereby warrant that I am of full age and have the right to contract in my own name. I have read the above authorization, release, and agreement, prior to its execution, and I am fully familiar with the contents of this document. This document shall be binding upon my signature.

Applicable Law

This contract shall be governed by the laws of the County of Maricopa in the State of Arizona and any applicable Federal law.
